Meeting the special needs of nursing home residents

The Borgess Extended Care Program is comprised of a team of nurse practitioners and a board-certified physician who work closely with residents, their families, referring physicians and the nursing home staff to provide continuous care. By taking a team approach, we are able to have more frequent contact with residents and closer working relationships with the nursing home staff.

Our wide range of services include a complete health history review and physical exam, routine check-ups, prompt treatment of illnesses and mild injuries, and hospice care if requested. Additionally, you’ll be pleased to know that the majority of our services are covered by Medicare.
