Calls for appointments are welcome between 8:00 am - 4:30 pm, Monday through Friday, except holidays. You do not need a referral from another physician to request a consultation or to make an appointment. Please call in advance for the most convenient scheduling. As an added convenience, you can request non-urgent appointments using the “Contact Us” section at the top of this page. Please allow us one business day to respond to your appointment request.
If you cannot keep an appointment, please call to cancel at least 24 hours in advance for regular appointments and 48 hours in advance for scheduled procedures. This allows us to schedule another patient for that time and serve everyone more effectively.
The nature of orthopaedic medicine requires that we treat certain injuries, pains, etc., on an emergency basis or spend additional time with patients who have developed unforeseen problems. Our clinic tries to keep these medical emergencies from delaying your scheduled appointment. If a delay in your appointment time is anticipated, we will make every attempt to keep you informed. Thank you in advance for your cooperation and understanding in these unavoidable situations.
Your First Visit
If you are a new patient or need orthopaedic treatment we suggest that you allow at least an hour for your visit with us. Please arrive fifteen minutes prior to your scheduled appointment so we can complete your registration process. You can assist in expediting this process by ensuring that you have all the necessary information listed below at your scheduled appointment, including:
Telephone Inquiries/Prescription Refills
Please call the office during normal business hours with any questions you have regarding your orthopaedic treatment needs. Routine prescription refills should be requested during your scheduled orthopaedic appointment with the physician. Refills may also be requested during normal business hours. When requesting a refill, please allow three days for the request to be processed so we may consult your physician. To expedite the process, please provide us with the pharmacy telephone number.
If your pain or injury is work related, please provide the office with an authorization letter from your employer or worker's compensation carrier including claim number or case number, contact person and their telephone number and appropriate billing address.
Auto Insurance Claims
If your injury is related to an auto accident, please bring your insurance policy information.
There will be a $10 charge for completing forms. Please allow 10 business days for the completion of the form.
